He separates the questions — are comas real? (yes, useful), is the GCS a good proxy? (yes), does it predict outcomes? (yes), is there a single 'General Factor Of Comatoseness'? (nobody thinks so, and it doesn't matter). His 'giant pot' model: many causes (heart/brain/liver disease) pour into one observable category that yields predictions, regardless of its internal causal structure; learning the internal structure can refine predictions but doesn't make the pot 'unreal.' The blood-pressure parallel (also an abstraction, also multifactorial and fuzzy, yet undoubted).
